We never want costs to stand in the way of quality treatment, which is why we are dedicated to providing our patients with eye care that is both effective and affordable. We do our best to provide a complete breakdown of costs to help our patient's understand their out of pocket expense before treatment is administered. We accept payment by cash, checks, and Visa, MasterCard, Discover and American Express credit cards. We also offer a discount for services when paid in full at the time of your visit as well as flexible in-house payment plans and financing through CareCredit.
Copays are due at the time of your appointment.
If you have any questions about financing or payment, ask us! We will thoroughly explain your choices and work hard to accommodate you. If you have a high deductible plan, we may ask for a down payment for services when you check-in.
Our office is committed to helping you maximize your insurance benefits. However, we believe the patient has responsibility to understand their individual policy including applicable copay and deductible amounts. Because insurance policies vary, we can only estimate your coverage in good faith but cannot guarantee coverage due to the complexities of insurance contracts.
When you first visit our office, bring your current insurance card with you. If you change insurance companies or join another employer’s plan, please be sure to let us know. If an insurance referral is necessary, please bring the completed form with you to your appointment. We do not re-bill any balance that has accumulated due to incorrect or outdated information being in our billing system. We are a preferred provider for most major insurances, including Medicare, Medicare +Blue, BCBSM, BCN, Aetna, HAP, Humana, Medicaid, Paramount, and Tricare. Effective January 1, 2018, we only accept VSP and SVS vision insurance plans.
We will fully attempt to help you receive full insurance benefits; however, you are personally responsible for your account, and we encourage you to contact us if your policy has not paid within a reasonable timeframe. If we are not contracted under your insurance, you will be given a copy of services and charges to file with your insurance claim form.
