Our office is committed to helping you maximize your insurance benefits. Please bring your current insurance card with you to each and every visit. If you change insurance companies or join another employer’s plan, please be sure to let our insurance department know. If your plan requires a referral from your primary care physician please be sure to obtain this prior to your appointment. ESA accepts most major insurances, including Medicare, Medicare + Blue, Aetna, BCBSM, Blue Care Network, HAP, Humana, Paramount, United Health Care, Priority Health, and Cigna. We currently only participate with SVS Vision plan. We will submit all insurance claims for you, however, you are personally responsible for your account. Because insurance plans vary, we can only estimate your coverage in good faith but cannot guarantee coverage due to the complexities of insurance contracts. Your estimated patient portion must be paid at the time of service including any office visit co-pay. We cannot re‐bill any balance that has accumulated if wrong information has been provided or previous information is outdated. If you have any questions, please don’t hesitate to ask.
